The Effect of Self-Care Education via Telenursing on Illness Uncertainty and Self-Efficacy in Elderly Patients with Colorectal Cancer

Introduction: Effective management of chronic diseases, especially during old age, requires continuous communication between patients and nurses. One of the key strategies in this field is the use of self-care-based educational programs, which can play a significant role in enhancing patients’ ability to make decisions and take care of themselves. In recent years, particularly with the spread of COVID-19, new methods of remote health education have gained increasing attention. Given the importance of this issue, the present study was conducted to examine the effect of self-care education via telenursing on illness uncertainty and self-efficacy in elderly patients with colorectal cancer.
Methods: This semi-experimental study with a control group was conducted with the participation of 50 elderly patients with colorectal cancer who had visited Imam Hossein Hospital in Tehran in 2023. Sampling was done through convenience sampling based on the study criteria. Data was collected using a demographic information form, Mishel’s Uncertainty in Illness Questionnaire, and the Cancer Behavior Inventory. Statistical analysis was performed using SPSS version 27. To examine differences and compare indicators between the two groups, independent t-tests, chi-square, and ANCOVA were used. A significant level of p < 0.05 was considered.
Results: The mean age of participants in the study was 64.4 years (SD = 4.5). Of the total participants, 54% were male and 46% were female. The mean and standard deviation of illness uncertainty before the intervention in the intervention group were 105.84 and 11.07, respectively, and in the control group, they were 102.68 and 13.9 (p = 0.21). After the intervention, the mean and standard deviation in the intervention group decreased to 88.12 and 11.01, and in the control group to 93.08 and 13.09. Although both groups showed a decrease in illness uncertainty, the change was not statistically significant (p = 0.15). Regarding self-efficacy, the mean and standard deviation before the intervention was 97.48 and 16.51 in the intervention group, and 104.32 and 18.25 in the control group (p = 0.57). After the intervention, these scores increased to 206.08 and 17.57 in the intervention group, and 164.92 and 31.27 in the control group. This increase was statistically significant in favor of the intervention group (p < 0.001).
Conclusion: The findings of this study showed that self-care education through telenursing can have a significant impact on improving the self-efficacy of elderly patients with colorectal cancer. Therefore, the continuous application of this method, along with regular telephone follow-ups, can be considered a practical and effective strategy for enhancing self-efficacy.
