Introduction: The transition from hospital to home is one of the most critical and high-risk stages in the care of older adults, associated with an increased likelihood of hospital readmission, functional decline, and patient dissatisfaction. Family support is recognized as an important factor in the success of this process; however, sufficient evidence regarding its role in the quality of care transition among Iranian older adults is lacking. Objective: This study aimed to determine the relationship between family support and the quality of care transition among older adults discharged from hospital and to explain this relationship based on Meleis’ Transitions Theory. Methods: This descriptive-correlational cross-sectional study was conducted in 2025 on 130 older adults aged 60 years and above who had been discharged from two hospitals in Khoy city. Data were collected using a demographic characteristics questionnaire, the Care Transitions Measure (CTM-15), and a family support questionnaire. Data analysis was performed using SPSS software version 26 and statistical tests including Pearson correlation, independent t-test, analysis of variance, and multiple linear regression. Results: The results showed a significant positive relationship between family support and the quality of care transition (r = 0.52, P < 0.001). Family support alone explained 27% of the variance in care transition quality (R² = 0.27, P < 0.001) and remained a significant predictor in the multiple regression model (β = 0.52, B = 0.48, 95% CI: 0.30–0.66, P < 0.001). In addition, all dimensions of family support showed significant positive correlations with the quality of care transition. Conclusion: Strengthening family support through active family involvement in discharge planning, caregiver education, and the development of post-discharge follow-up services may help enhance the quality of post-discharge care.
